Only those districts/charter organizations whose 2025-2026 State Seal of Arts Proficiency Application was received on or before 08/28/2025 are required to complete this form.
The 2025-2026 Arizona State Seal of Arts Proficiency Implementation Guide was revised on 08/28/2025 to remove information unrelated to implementation of the Seal of Arts Proficiency. The Seal of Arts Proficiency objectives, requirements and 25-26 timeline have not changed.
