These resources are intended to give suggestions, supply sample forms, and assist with clarification of requirements. These sample forms can be used in your Work-Based Learning Programs. Each of these forms are Word Documents in template form that you can download and adjust to fit the needs of your district and school. Please be advised that these are samples that need to be reviewed by your legal counsel and board for adopted use if required by your administration.
